Hired by the purchasing owners/investors of nine previously bankrupt centers, Transitional Care Management took over the operations of:
  • Three skilled nursing centers
  • Four intermediate care centers and
  • Two specialized mental health rehabilitative facilities.

KEY ACCOMPLISHMENTS

Transitional Care Management:

  • Formed an agreement for collections related to the prior ownership, resulting in collections of $8.5MM.
  • Initiated a fully integrated electronic health records system.
  • Launched payroll and human resource systems.
  • Centralized admissions functions throughout the portfolio of communities.
  • Secured relationships and created new referral streams with nationally known behavioral health referral sources, local hospitals, and prime agencies.
  • Created innovative, nationally recognized, and award-winning Life Skills programming to support resident reintegration into the community.
  • Improved training and education through SAMHSA Center of Excellence educational opportunities, Transitional Care University courses, and a formal Administrator in Training career path training.
  • Developed a full complement of company-wide support featuring nationally recognized clinical/nursing, therapy, behavioral health, and admissions leaders.
  • The current cash flow for previously bankrupt centers is at $5MM annually.
