A county-owned, century-old, 224-bed skilled nursing facility in Libertyville, IL

Transitional Care Management earned the request for a proposal to transform an existing 100+-year-old, county-owned nursing home into a viable, 21st-century care center while simultaneously improving the quality of care, staff recruitment and training, systems and process, and financial viability of the existing center in the interim.

KEY ACCOMPLISHMENTS

Transitional Care Management:

  • Worked with the county, Illinois Department of Public Health, investors, business partners, and contractors to select a more appropriate location to build a new 21st-century care center for the residents of Lake County
  • Collaborated with key stakeholders to design, build, and secure licensure for the new center
  • Expanded the new center’s offerings to include memory care, skilled nursing, and post-acute transitional care
  • Managed the existing center’s day-to-day operations and patient care
  • Converted to an electronic medical records system
  • Renegotiated and reduced vendor-related expenses by over 20%
  • Maintained regular communication with the Advisory Board
  • Secured post-acute network status with all area healthcare systems, key managed care, commercial insurance, and worker compensation contracts
  • Upon completing the new center, over 100 residents, staff, and operations were transferred in one day.
