CHIEF EXECUTIVE OFFICER
Transitional Care Management (TCM) was founded by Brian Cloch in 2009. With nearly 30 years of experience in developing, financing and managing senior living and rehabilitation facilities, Cloch has developed a winning plan and a proven team for building and managing skilled nursing and transitional care facilities.
DENISE NORMAN
PRESIDENT
An experienced team leader with an extensive program, business and staff development background as well as financial management experience, Denise began her career as a Physical Therapy Assistant. Her hands-on patient care experience and business savvy have earned her several customer service and performance awards.
MIKE FILIPPO
CHIEF FINANCIAL OFFICER
An experienced nursing home owner and operator, Mike’s accomplishments include direct responsibility for entire operations of multi-facility long-term care groups, including physician recruitment, development, and implementation of specialty clinical programs, regulatory performance, and reimbursement strategies.
MICHELLE STUERCKE
CHIEF CLINICAL OFFICER
Michelle Stuercke, RN, MSN, DNP, MPH, LNHA assists with developing and implementing specialty clinical programs at TCM managed properties. A licensed nursing home administrator, Michelle earned her Master in Public Health degree from Roosevelt University and Doctorate in Geriatric Nursing from Rush University. She offers significant program development, mentoring, regulatory and financial management expertise.
CHARLES A. ROSS
CHIEF STRATEGY OFFICER
Charles Ross is an experienced strategic post-acute care operator and business development professional with a history of maximizing results and creating high-level partnerships across the healthcare continuum. He works closely with hospitals, home health, hospice, and skilled nursing, including a focus on managing new purpose-built stand-alone post-acute care facilities.

Beth leads a team of TCM staff accountants, patient account representatives, managed care liaisons, and accounts payable.
